New Patient Forms

Patients who are new to Cottesloe Sports Physiotherapy are required to fill out our Patient Medical History form.

Your First Visit

At Cottesloe Sports Physiotherapy all our physiotherapists conduct their consultations in a professional manner. We endeavor to make you comfortable at all times while you are in pain.

  1. A thorough assessment will be performed. This usually involves reviewing your past injury history. If you have any current or old MRI or CT scans/x-rays, please bring them along. This assessment will be followed by a clear explanation of your condition. It is at times necessary to refer you on to a Doctor or Specialist for further treatment or investigations. Our Physiotherapists are well connected with the best network of specialists in both Perth and the Eastern States of Australia.
  2. A treatment plan will be implemented with regular assessment to measure the success of the treatment.
  3. An exercise program, during the initial or subsequent consultation, will also be implemented to promote functional improvements and self-management.



Subsequent Visits

These involve review and progression of treatment. As Physiotherapy optimises the healing process your body will start to tolerate increased loads, demonstrate better alignment and improved movement patterns resulting in improved function and the absence of the body’s warning signal for damage control-pain!

  1. Progressions of your exercise program will be performed throughout the different stages of your improvement. This is to ensure a structured and speedy return to normal function.
  2. A return to work, sport and daily activities/chores plan is implemented and followed through.



Musculoskeletal Screening

Your coach may have referred you to Cottesloe Sports Physiotherapy for a musculo skeletal screening. What is it?

Over the years, Sports Physiotherapists treating Australia’s elite athletes in all sports ranging from the Olympic sports to AFL have developed a series of tests relevant to their particular sport that can be completed in the physiotherapy clinic. This determines two important factors:

  1. The athletes risk of injury, and
  2. What the athlete must do to improve their performance

Over recent years, Cottesloe Sports Physiotherapy’s Practice Principle, Brett Slocombe, has been heavily involved in streamlining the screening process for two Australian Olympic Sports - swimming and kayaking. Each year he screens athletes from the Australian Teams in both sports in addition to all WAIS (Western Australian Institute of Sport) athletes. This information is used along with direct observation such as on-water or underwater filming performed by the Biomechanics Department. Other input from coaches can then all combine to optimise the athlete’s body for performance and injury prevention.

The Musculoskeletal Screening service is also available to the general public and is not only limited to those two Olympic sports.

Screenings take 45 minutes, but please leave an hour to be safe. Athletes should wear appropriate clothing to allow full assessment. For men, shorts and for women shorts and crop top or bathers is appropriate. You will be given a full list of findings and a list of recommendations/exercises addressing the issues that have been identified. As a general rule, follow up from an initial musculo skeletal screening will occur 3 months after the initial screening to re-assess and determine if the exercises/recommendations are working. Some hands on physiotherapy (manual therapy) may be required, to improve your muscle activation patterns, posture or spinal alignment.

The screening results will be sent to your coach on request complete with photos of special screening findings and exercises.

Appointments

Appointments for Physiotherapy treatment may be made by telephoning one of our receptionists on 9286 1000 or by completing the form in the contact us section of this website. We will usually be able to arrange your appointment with a physiotherapist for the same or following day.

Patients in severe, debilitating pain will always be seen immediately.




Cancellations

It is our preference to be able to provide patients with time slots that may have become available due to cancellation. If you need to change the time of your appointment or cancel it, 24 hours notice is required. Full cancellation fees may apply.
